Integrated Behavioral Health

Medical and Behavioral Consultation for Health Promotion and Disease Management:

A Team-Based Approach to Primary Care

What is Integrated Behavioral Health?

Integrated Behavioral Health services assist patients when habits, behaviors, disease progression, stress, worry, or emotional concerns about physical, medical, or other life problems interfere with daily life and/or overall health. Services may be consultative, individualized, provided in groups or shared medical groups.

What is an Integrated Behavioral Consultant?

Integrated Behavioral Health Consultants (IBHCs) at CFHC, Inc., are master and doctorate level clinicians who work with the patient and his/her Primary Care Provider (PCP) to evaluate the mind-body-behavior connection.  IBHCs provide brief, solution-focused interventions to help patients improve their health and address other issues affecting their lives.

What Should I Expect When I See the IBHC?

The IBHC will ask specific questions about physical symptoms, emotional concerns, behaviors, and discuss how all of these might be related. Appointments are no longer than 30 minutes. In addition, the IBHC will work closely with the patient and his/her Primary Care Provider.

How is Integrated Behavioral Health Different from Mental Health?

Integrated Behavioral Health focuses on the patient's overall health and well-being with the end goal of improving health outcomes through brief interventions the patient can use in his or her daily life. Mental health care, such as traditional psychotherapy, focuses on the patient’s chronic mental illness or substance abuse. The IBHC will help coordinate services to specialty care providers in the community for mental health care.

The IBHC can help patients reduce symptoms associated with chronic medical conditions to improve functioning and help the patient better cope with these conditions. The IBHC will help the patient develop skills to effectively manage emotional or behavioral difficulties through behavioral change plans, lifestyle modifications, resource building, and targeted person-centered interventions. Integrated Behavioral Health services are often sought for:
