Ways to Sign Up
- Click the MyChart button above. Use the activation code from your Care Team, your date of birth, and the last 4 digits of your social security number to register on your own.
- At the health center with an activation code and help from staff.
- Download the MyChart – By EPIC app for your smartphone. Once you’ve downloaded the app, search for “Carolina Family Health Centers,” then log in with your MyChart username and password to register on your own.
