New Patient
Carolina Family Health Centers, Inc. welcomes new patients of all ages. To schedule a new patient appointment, simply call the center most convenient for you and your family.
Below is important information to prepare you for your first appointment. On the day of your appointment, arrive 30 minutes early to register.
Please bring the following:
- A valid identification card
- An insurance card if you have one
- Medications you are currently taking
- Proof of income if you wish to apply for the sliding fee discount program
