Please call any of our centers and ask to schedule a new patient appointment.

Do I have to be a patient to use the pharmacy?

Patients must be established with our medical and/or dental centers to access our pharmacy.

Do I have to be a medical patient to use dental services?

No, you do not have to be a medical patient to use our dental services, however, primary medical care and oral care are both important for overall health. We would appreciate the opportunity to serve you in both areas.

What should I bring to my first appointment?

On the day of your first appointment, please arrive 15 minutes early to register. Please bring the following:

• A valid identification card
• An insurance card if you have one
• Medications you are currently taking
• Medical records if you are transferring from another medical provider

If I am sick and need urgent care, do you accept walk-in appointments?

We do accept walk-in appointments but suggest that you call first to help reduce wait times.

Are Carolina Family Health Centers free clinics?

No, we do not provide free services. We offer flat rate and sliding scale discounted fees for services based on your total household income.

If I don't have insurance, can I be seen by a provider?

Yes, we accept all patients, regardless of whether or not they have a health insurance policy. We offer a sliding fee discount program for those without insurance that is based on household income.

If I have insurance, can I still use your services?

Absolutely, we see all patients, regardless of insurance status.

What is the Sliding Fee Discount Program?

The Sliding Fee Discount Program is based on household income and the number of dependents in the household. Depending on where patients rank on the scale, their medical fees for our services can be discounted. We also offer flexible payment plans to assist with managing healthcare costs.

Do you offer transportation assistance?

Yes, we offer transportation assistance to and from medical and dental appointments. When you call to schedule your appointment, please let the call attendant know that you need this service.

We also offer free medication delivery in our pharmacy. Ask a pharmacist about the service when you call for your refill.

What is a Federally Qualified Health Center (FQHC)?

FQHCs, as defined by the Health Resource & Services Administration (HRSA), are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in underserved areas. They must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients.

What is a Patient Centered Medical Home (PCMH)?

According to the Agency for Healthcare Research and Quality, patient-centered primary care is defined as care that is relationship-based with an orientation toward the whole person, and that includes partnering with patients and their families to understand and respect each patient’s unique needs, culture, values, and preferences. Care that is patient-centered also supports patients in learning to manage, organize, and participate in their own care at the level the patient chooses.
